You may print the following forms, complete them and fax, email, mail or bring them into City Hall to expedite your transaction(s). Please take note of any form(s) that require photo identification.

Should you have any specific questions, instructions or comments, you may type them in the body of the email that will transmit your form. Also, before sending, be sure to “REQUEST A DELIVERY RECEIPT.” You are also encouraged to call City Hall the day after you send in your form to verify that it was received.
